When it is needed

An annex completed by the entity entrusting work is attached to an application for (Art. 106 para. 2 of the Act on Foreigners):

  • a temporary residence and work permit,
  • a temporary residence permit for the purpose of performing work in a highly qualified profession (EU Blue Card),
  • a temporary residence permit for the purpose of long-term mobility of an EU Blue Card holder,
  • a temporary residence permit for the purpose of running a business - where the purpose of stay is work on the management board of a limited liability company or a joint-stock company that the foreigner established or in which the foreigner took up or acquired shares, running the affairs of a limited partnership or a limited joint-stock partnership as a general partner, or acting as a commercial proxy (prokurent).

Similar annexes are completed by the organiser of an internship, the host entity of a volunteer and the institution providing studies - for permits for interns, volunteers and for studies. They work in the same way in MOS.

What it contains

  • the foreigner's details,
  • the details of the entity entrusting work and of the user employer,
  • information about the work: the position or type of work, the occupation according to the classification of occupations and specialisations, the place of work, the legal basis, the working time, the remuneration, the scope of duties, the period of entrusting work and the PKD subclass,
  • the employer's statement that it has not been convicted of certain crimes and offences,
  • a statement that the data is true.

The template of the annex is set by the regulation of the Minister of the Interior and Administration (MSWiA) of 10 April 2026 on the application for granting a foreigner a temporary residence permit.

How it works in MOS

  1. While filling in the application, the foreigner enters the employer's e-mail address and confirms it with the "Add" button.
  2. After filling in the rest of the application, the foreigner chooses "Save the application and send the form to the employer". The application receives the status "Awaiting completion of the annex by the employer" - this is not yet the submission of the application.
  3. The employer receives an e-mail with a link, fills in the form online, signs it and sends it in MOS. The annex is automatically added to the application.
  4. The foreigner receives an e-mail saying that they can finish the application - they sign it and send it.

For the employer: signature

The annex is signed by the entity entrusting work or a person acting on its behalf - with a qualified electronic signature, a personal signature or a trusted signature (Art. 106d para. 4). Where the employer is a company or another organisational unit, the statements are made by the person acting on its behalf (Art. 106 para. 3).

Deadline: the application must be submitted no later than on the last day of the legal stay, and the foreigner cannot send it until the employer sends back the annex. Send the link to the employer well in advance and agree a date with them.

Change of a temporary residence and work permit

An application to change a temporary residence and work permit is submitted in paper form, not in MOS (Art. 120a para. 1). See: applications outside MOS.
