What you must attach straight away
- a current photograph (a JPG file),
- a scan of all pages of a valid passport - and exceptionally, when you do not have a passport and cannot obtain one, of another identity document,
- the annexes required for the given purpose of stay, for example Annex No. 1 from the employer.
Basis: Art. 106d para. 1 of the Act on Foreigners. For a permit for an EU long-term resident, the MOS portal additionally lists a legal title to the premises.
Photo requirements
According to the regulation of the Minister of the Interior and Administration (MSWiA) of 10 April 2026, the photo:
- is in colour, sharp and undamaged,
- was taken no earlier than 6 months before the application is submitted,
- has the proportions of a 35 x 45 mm photo,
- shows the face from the top of the head to the upper part of the shoulders - the face takes up 70-80% of the photo - against a plain light background,
- shows you facing the camera, with your eyes open and not covered by hair, with a natural facial expression and a closed mouth, in a natural skin colour,
- clearly shows the eyes, especially the pupils, and the line of the eyes is parallel to the top edge of the photo.
File: JPG format, resolution of at least 684 x 883 pixels (width x height), size up to 2,5 MB.
The photo goes on the residence card. If the system rejects the photo, you will not be able to send the application. If the photo passes but does not meet the requirements, the voivode will summon you to send a proper one - and if you do not, the application will be left without examination.
Files with documents
- a single file may be up to 10 MB, and all the files together up to 50 MB,
- you attach scans of paper documents in PDF format, legible enough that the content raises no doubts.
Other documents
You may attach documents confirming the data in the application and the purpose of stay to the application as files (Art. 106d para. 2). The law does not require this: the voivode will in any case summon you to submit them, setting a deadline of not less than 14 days (Art. 106f). If you attached scans, the voivode will summon you to show the originals. According to the MOS portal, attaching documents earlier may speed up the proceedings.
