With every application
You attach to the application in MOS at once (Art. 106d para. 1 of the Act on Foreigners):
- a current photo,

We describe the technical requirements in the guide photo and attachments. The MOS portal also lists proof of payment of the stamp duty for the permit and of the fee for issuing the residence card. Amounts: fees.
Documents confirming the purpose of stay

According to the MOS portal, documents in a foreign language are presented with a sworn translation into Polish.
